My favourites (only including those that I've seen in person, there are many more I'd love to see):

Sumatran Orangutan/Siamang- Melbourne Zoo, I love the use of sway poles, I think more zoos should use them in orangutan enclosures.

Chimpanzee- Monarto Zoo, very large enclosure with live trees (I'm surprised how long they've survived actually) as well as climbing frames and ropes.

Western Lowland Gorilla- Melbourne Zoo, my favourite great ape exhibit that I've seen so far, beautiful and lush with plenty of cover for the gorillas and easy for the gorillas to get out of public view if they wish.

Dusky Leaf Monkey- Adelaide Zoo, exhibit consists mainly of a stunning and enormous fig tree, the enclosure is shared with Malayan tapirs.

These are the main ones that have stood out to me. :)

    Some of the best primate exhibits I've seen;
Bronx Zoo's Scrub Forest Exhibit: http://www.zoochat.com/547/madagascar-scrub-forest-exhibit-296453/
Beardsley Zoo's Goeldi Monkey Exhibit: http://www.zoochat.com/1133/new-world-tropics-building-287396/

    Bronx Zoo's Gorilla Exhibit: http://www.zoochat.com/547/congo-gorilla-forest-241456/
    Busch Gardens Tampa's Myombe Reserve: http://www.zoochat.com/551/myombe-reserve-chimpanzee-exhibit-287758/, http://www.zoochat.com/551/myombe-reserve-gorilla-exhibit-see-gorilla-287761/
    Bronx Zoo's Baboon Reserve: http://www.zoochat.com/547/baboon-reserve-296516/
    Bronx Zoo's Gibbon Exhibit: http://www.zoochat.com/547/bronx-zoo-jungle-world-41122/
I haven't been to many zoos so my list is somewhat limited. Also, I can't remember my visit to Disney's Animal Kingdom well as it was a very long time ago, so that can't count for me.

I quite like the Smithsonian National Zoo's Ring-Tailed and Red-Fronted Lemur exhibits.
    http://www.zoochat.com/612/national-zoo-lemur-island-43665/
    http://www.zoochat.com/612/very-good-lemur-exhibit-94700/

Also, while not the most beautiful and naturalistic exhibit, I like how the SNZ has the O-Line to allow their Bornean Orangutans to travel one the Great Apes exhibit to the Think Tank exhibit.
    http://www.zoochat.com/612/o-line-269874/
    http://www.zoochat.com/612/kiko-orangutan-o-line-213288/

In the SMZ's Amazonia exhibit the upper floor is one big free flight rainforest with monkeys like Goeldi's Monkey.
    http://www.zoochat.com/612/goeldis-monkey-296065/

Even though they've already been said, I do like the Bronx Zoo's Congo Gorilla Forest, their langur exhibits, the lemur exhibit in the Madagascar! exhibit, their Gelada Baboon exibit, and their huge rainforest room that holds Northern White-Cheeked Gibbons.

The Houston Zoo is building a really nice Gorilla exhibit. It's also going to have Red River Hogs with the Gorillas. It's opening next year.

It looks like it's going to be pretty nice. Houston Zoo is one of the few zoos I can visit on a semi-regular basis, so I'm really looking forward to it.

For Chimps I would say Burger's Zoo - the Netherlands. Not only VERY good but also world-famous by the books about it by Frans de Waal :)

The inside enclosures are not that spiffing, but especially the outdoor part has been in the top league for over 40 years now

Yes, Burgers chimps enclosure is excellent(outdoors). Its the same story with Apenheul's Gorillas, outdoors is terrific, indoors not up to the same standard.

Burgers outdoor Gorillas is not bad either and the group has really expanded recently.

Baboon Reserve of Bronx Zoo is the best zoo exhibit I have ever seen; and the red-shanked douc and proboscis monkey exhibits of Singapore Zoo are also fantastic.

I love Baboon Reserve! I just hope they get more Geladas soon because it's beginning to feel a bit empty.

I personally think many of the best primate exhibits out there are at Vallée des Singes and Apenheul Primate Park. They started the trend of natural, big, walk-through exhibits for primates with live trees. Here are a few of my favourites:
Lemur Walkthrough (could use a few more species, however) at Apenheul http://www.zoochat.com/159/lemur-walkthrough-apenheul-30-05-12-a-276024/
"Monkey tree-top trail" with white-faced saki, Goeldi's monkey, red howler monkey, Emperor tamarin & silvery marmoset at Apenheul http://www.zoochat.com/159/red-howler-monkeys-275313/
Northern white-cheeked gibbon island at Apenheul (could use a little more indoor space, but overall a brilliant habitat) http://photos.zoochat.com/large/apenheul_30-21404.jpg
Gelada exhibit at Tierwelt Herberstein http://photos.zoochat.com/large/dsc065871-137228.jpg
Chimpanzee exhibit at Vallée des Singes http://photos.zoochat.com/large/vds_chimps-94871.jpg
